## About This Service

Collaborative Care adds behavioral health support your treatment at The Center. A dedicated care manager checks in with you, and your entire team works together to help with stress, worry, low mood, or anything else you're facing.

## How It Works

Support that fits into the care you already get

#####  Your provider refers you

Your care team talks with you about the program and, if you'd like to join, adds it to your care. There's no new clinic to visit.

#####  A care manager checks in

A behavioral health care manager gets to know you, checks in regularly, often by phone, and helps track how you're feeling over time.

#####  Your team stays in sync

A psychiatric consultant reviews your progress with your care manager and doctors, and your support is adjusted if something isn't working.

## Behavioral Health Support Into your Care

The Center partners with Adjuvant to bring behavioral health support into your care.

Your behavioral health care manager is your main point of contact. They check in regularly, offer support, and connect you to community resources.

A psychiatric consultant reviews your care with the team behind the scenes and recommends adjustments if needed.

## Commonly Asked Questions

#####  Is this the same as therapy?

Not exactly. Collaborative Care is ongoing support and check-ins from a care manager, guided by a psychiatric consultant and your doctors. If you need more specialized care, your team can help connect you to it.

#####  Do I have to come in for extra appointments?

Usually not. Most check-ins happen by phone, and your support is coordinated with the care you already receive at The Center.

#####  Will it cost me anything?

Collaborative Care is covered by Medicare and most insurance plans. Copays or coinsurance may apply. Ask us and we'll help you understand your coverage before you begin.

#####  What if I'm not sure i need it?

That's okay. Talk with your care team about how you're feeling. You can join at any point in your treatment, and you can stop at any time.

#####  Is this for emergencies?

No. If you are in crisis or thinking about harming yourself, call or text 988 (Suicide & Crisis Lifeline) or call 911 right away.
