Collaborative Care adds behavioral health support your treatment at The Center. A dedicated care manager checks in with you, and your entire team works together to help with stress, worry, low mood, or anything else you're facing.
How It Works
Support that fits into the care you already get
Your care team talks with you about the program and, if you'd like to join, adds it to your care. There's no new clinic to visit.
A behavioral health care manager gets to know you, checks in regularly, often by phone, and helps track how you're feeling over time.
A psychiatric consultant reviews your progress with your care manager and doctors, and your support is adjusted if something isn't working.

The Center partners with Adjuvant to bring behavioral health support into your care.
Your behavioral health care manager is your main point of contact. They check in regularly, offer support, and connect you to community resources.
A psychiatric consultant reviews your care with the team behind the scenes and recommends adjustments if needed.
